Looking to do a survey or questionnaire ( only amoungst 30 people ) to get their opinions/insight. The topic is project management and collaboration. Its to get an overview of what project management , what tools people use ( whether software based, paper based , desktop based , online based ). Why they use those tools , any advantages they find in their current tools , any disadvantages they find in their current tools , what they would desire from a perfect solution.

Can anyone suggest questions I can ask or point me to a already done survey/questionnaire I can use ?

I am doing a project that requirs materials and tools needed for the apothecary job of the 18th century! PLEASE help!
I will give 10 points for the best answer. My friend and I are doing a replica of how it would be. We will dress up and do a little show type thing for school.
